We do not collect health information through this website

This is deliberate, and it shapes everything below.

Our forms ask only what we need to call you back. We do not ask for a diagnosis, a treatment history, or an insurance member number through the website. When you request an eligibility check, we ask for your insurance details on the phone, not in a web form. If your plan requires your written permission for us to verify benefits on your behalf, we send that to you separately, through a service that is contractually bound to protect health information.

Please do not send medical details, diagnoses, or insurance card numbers through our website forms or by email. If you have already sent something like that, contact us at [PRIVACY-CONTACT] and we will remove it.

What we collect

When you fill in one of our forms, we collect what you enter:

  • Contact form — your name, email address, phone number, and your message
  • Eligibility form — your name, phone number, and when you would prefer we call [CONFIRM-ELIGIBILITY-FIELDS]
  • Careers form — your name, contact details, and the résumé you upload

Information collected automatically

Who else handles your information

We use a small number of service providers to run the website. Each has access only to what it needs.

  • GoDaddy hosts the website and its database.
  • Fluent Forms powers our forms; submissions are stored in our website database.
  • Akismet, a service operated by Automattic, Inc., filters spam. When you submit a form, the information you entered and your IP address are sent to Automattic’s servers in the United States so the submission can be checked against known spam patterns. Automattic states that it does not sell this data and that most spam-related data is deleted automatically within two weeks to ninety days. Their notice is at akismet.com/privacy.
